The patient is a 43-year-old black female who presented to the office with extremely large breasts even for patients who presented to the office for breast reduction. She complained of neck, back and shoulder pain. Due to the exceptionally long distance between the nipple and the base of the breasts, i.e. 31 cm, most plastic surgeons would have opted for amputation of the nipple and grafting which renders the nipples insensate. However, the patient wanted sensate nipples at the end of her operation. We therefore designed an operation tailored to her needs which called for an exceptionally wide pedicle to maintain good perfusion to the nipple. A total of 2520 g breast tissue from the left breast and 2300 g breast tissue from the right breast were removed. In total, 4820 g or 10.6 pounds of tissue was removed in total. The operation was a success and the patient was extremely happy with the results. Her back pain, neck pain and shoulder pain were instantly relieved after surgery. She said this was the best decision she ever made. It took her 3 years to make this decision. She wished she had made the decision earlier.
